Background Investigation Process

The background investigation process is changing slightly and I wanted to keep you abreast of what is needed. The major difference in the process is that the SF85 form that has been completed by the employee and sent to HR with the other forms is now being done online in the e-QIP system. This process is being changed by NHQ because OPM no longer accepts the hard copy of the SF85 and going forward must be done online in the e-QIP system. Below is a list of what is needed from the supervisor and employee and sent to the attention of Human Resources Assistant Jean Howard at the Maryland State Office in Annapolis for a complete package.

FORMS/ITEMS NEEDED FROM SUPERVISOR/EMPLOYEE:

  • $121.00 check for background investigation (Non-Federal employees only), (rate effective as of 10/01/2008);
  • Completed and scored/passed Computer Privacy & Security Awareness Test (computer access will NOT be granted until this passed test is provided);
  • IRM-02 form signed & dated by employee & their supervisor, office manager, or contracting representative;
  • IRM-03 form completed & signed by the District Conservationist;
  • OF-306 completed & signed by employee;
  • OF-612 completed & signed by employee;
  • 2 SF-87 (fingerprint cards);
  • Hardcopy of SF85 (printed by employee in e-QIP);
  • Hardcopy of signature & certification pages in e-QIP (printed by employee in e-QIP).

The $121.00 check, Completed/Passed Computer Privacy & Security Awareness Test, IRM-02 form, IRM-03 form, form OF-306, form OF-612, and 2 SF-87 (fingerprint cards) can be sent together to Jean Howard. After she receives these items, she will fill out an e-QIP invite form and provide it to NRCS NHQ. Soon thereafter, the employee will get an email from NRCS NHQ inviting them to do the SF85 online in e-QIP. The employee should complete it online and print out a hard copy of the SF85 and signature & certification pages in e-QIP and send those hard copies to Jean at the MD State Office. Those last items will be put with the other forms needed and she will send the employee's entire complete package to HQ for processing. This is a very basic summary of what is needed.
